Addressing Disparities in Diabetes Care Among Language Minority Patients

Goals

The objectives of the study are to improve diabetes care and self-management for Spanish-speaking patients by:

  • Providing patients with tailored, language appropriate information and advice to better manage their self-care
  • Improving communication between those patients and their clinician teams
  • Enabling patients’ informal care-giving networks to fill gaps in self-management support resulting from inadequate reimbursement and limited primary care services
